Transitions Living provide supported accommodation for vulnerable young people who are ready to move on from their foster or residential placement or who have significant issues in respect to making the transition towards independence. We support young people over the age of 16 and up to 25 if still being helped by the responsible authority with education and training.

At Transitions living we believe that each young person is an individual and therefore their needs should be planned for appropriately in order to support them in the best possible way. We provide a range of services such as supporting young people with activities of daily living to providing emotional support.

At Transitions Living we are committed to improving the lives of the young people we support and providing a secure base for them to make to the transition to independent living.

 

Who Are We

Transitions Living is managed by Brenda Finlay-Musonda who has over 15 years experience of working with children and young people and therefore brings a diverse range of skills and experience to the service. Brenda has a Masters in Systemic Psychotherapy and trained at the Tavistock. She is also a qualified Mental Health Nurse.
